HEALTH SERVICES DEPARTMENT

We at Elko County School District are fully committed to our students’ health. We look after each student’s physical and mental wellbeing while they are in our care and work together with families and the community to ensure the health of our community’s children.

Did you know...Beginning July 1, 2024, all student-athletes will be required to have an annual physical using the new forms to meet NIAA requirements.

Annual Health Information

Please use the following links to communicate important health information with your child's school. These must be completed annually. The following consent form must be signed by a parent/guardian prior to your student receiving over the counter medications from the Health Office.

*If your child requires specialized care from the School Nurse please contact the Health Office for which your child is enrolled.

Medication at School

Certain health problems require that students take medication during school hours. For the safety of our students, we require that medication be kept in the health office at all times, including over the counter medications(OTC).

Medication must be in its original, properly labeled container.

A signed consent form giving specific directions for use must accompany medication (including OTC) from the parent AND healthcare provider.

Immunization Information

State law requires students to complete all required immunizations before school starts. To protect our students, we cannot permit students who have not provided proof of required immunizations to attend school or ride the bus on the first day of classes. We do our very best to notify parents by phone &/or mail if your child needs additional immunizations before the new school year. Please submit any immunization updates to the school Health Office for which your child is enrolled.

See the list of required vaccinations and resources below. Reach out to your child's school with questions or concerns. Thank you!
